• Please describe the type of animal the complaints are in regard to (cat or dog) and describe the animal in as much detail as possible.  Also report if the animal is wearing a collar with or without tags and the demeanor of the animal, as well (i.e. vicious, friendly, etc.).

• If the complaint is in regard to more than one animal, please describe each animal involved.

• Describe briefly the problem with the reported animal.

• If the animal(s) is owned, please provide as much information as to whom the possible owner(s) is and an address for the possible owner, if available.

• Please advise in the space provided 1) yes, if this is the first reporting of the animal, or 2) no if it is not the first reporting of the animal.  If this is not the first reporting of the animal, please provide the date on which it was first reported.
